Eligibility & How It Works

ELIGIBILITY

To request testing through Crisis Box Direct Lab, you must:

• Be at least 18 years old.
• Request testing for yourself.
• Provide accurate identity, contact, eligibility, shipping, and order information.
• Be physically located in an eligible United States jurisdiction.
• Meet all requirements of Evexia Diagnostics, the independent ordering provider, and the performing laboratory.
• Agree to the Terms of Use, Privacy Policy, Refund, Cancellation & Replacement Policy, and Lab Testing Consent.
• Understand that testing is self-pay and is not submitted to insurance.

Under Evexia’s current requirements, its Internal Physician Network is not available for non-licensed orders in New York, New Jersey, or Rhode Island. Availability may change and is confirmed during ordering.

Evexia currently does not provide services to recipients or beneficiaries of Medicare, Medicaid, or another governmental healthcare program. Charges may not be submitted to governmental or commercial insurance for reimbursement.

Testing is not intended for emergency symptoms, active medical emergencies, or situations requiring an immediate diagnosis or treatment decision.

Submitting a request does not guarantee that a provider will authorize the order or that the laboratory can perform the test.

HOW IT WORKS

  1. SELECT YOUR TEST

Review the test description, specimen collection method, preparation requirements, biomarkers, limitations, price, estimated turnaround time, and eligibility information.

  1. CONTINUE TO SECURE ORDERING

Select “Continue to Secure Ordering.” You will leave the Crisis Box Direct Lab website and continue to an ordering platform operated by Evexia Diagnostics.

  1. COMPLETE THE REQUIRED INFORMATION

Enter all required identity, demographic, eligibility, and payment information directly into the designated ordering platform.

Do not send this information through the general Shopify contact form or ordinary email.

  1. PROVIDER AUTHORIZATION

When required, an independent licensed provider may review the request solely to facilitate the laboratory order. This is not a comprehensive medical consultation or an ongoing medical-care relationship with Crisis Box Direct Lab.

  1. RECEIVE YOUR REQUISITION OR KIT

For collection-center tests, you will receive the requisition and instructions needed to visit an eligible collection location.

For at-home tests, an authorized fulfillment provider may ship a specimen kit to the eligible address provided.

  1. COMPLETE SPECIMEN COLLECTION

Follow all preparation, timing, identification, collection, storage, and shipping instructions provided for your test.

  1. ACCESS RESULTS

Results are made available using the method designated by Evexia Diagnostics or the performing laboratory.

Crisis Box Direct Lab does not provide medical result interpretation. Discuss results, symptoms, and health concerns with a qualified healthcare provider.
