Crisis Box Direct Lab offers both laboratory tests collected at an authorized collection center and tests that use an at-home specimen kit. The fulfillment process depends on the individual test.

  1. COLLECTION-CENTER TESTS

Tests requiring a blood draw or collection at a Labcorp Patient Service Center generally do not include a physical product shipped by Crisis Box Direct Lab.

After the order is authorized, the customer receives access to a laboratory requisition and collection instructions through the designated email or secure platform.

Customers are responsible for confirming the location, operating hours, appointment requirements, identification requirements, and availability of the selected collection center before traveling.

  1. AT-HOME SPECIMEN KITS

For eligible at-home tests, the specimen kit may be fulfilled and shipped by Evexia Diagnostics, the performing laboratory, or another authorized fulfillment provider.

Available shipping methods, eligible locations, charges, and estimated delivery times may vary by product and destination.

A complete and accurate residential shipping address is required. We are not responsible for delays, loss, or additional charges caused by an incorrect or incomplete address provided by the customer.

  1. DELIVERY ESTIMATES

Processing, delivery, specimen transportation, and laboratory turnaround times are estimates and are not guaranteed.

Delays may occur because of provider review, inventory, weather, carrier operations, holidays, laboratory workload, specimen quality, geographic restrictions, or events outside our control.

  1. RECEIVING AND INSPECTING A KIT

Inspect the package promptly after delivery. Do not use a kit that appears opened, damaged, incomplete, contaminated, or expired.

Report a problem promptly to orders@cbxdirectlab.com with the order number and photographs when appropriate. Do not email photographs of specimens, identification documents, or medical records.

  1. SPECIMEN COLLECTION AND RETURN

Follow only the instructions supplied with the specific kit. Requirements differ by specimen type and laboratory.

The customer is responsible for proper preparation, collection, labeling, storage, packaging, and return shipment. Use only the return method included or authorized for the kit.

Do not mail a specimen using an unapproved carrier or packaging method.

  1. LOST, DAMAGED, OR REJECTED SPECIMENS

If a kit or specimen is lost or damaged during transportation, contact us so the shipment can be investigated with the responsible fulfillment provider or carrier.

Replacement or recollection eligibility depends on the cause and the policies of the performing laboratory. Additional charges may apply when the customer did not follow the provided instructions.

  1. ADDRESS AND GEOGRAPHIC RESTRICTIONS

Kits and laboratory services are available only in eligible United States locations. Orders cannot be fulfilled in a location where the test, provider authorization, laboratory service, or shipment is prohibited or unavailable.
