Terms of service
LAST UPDATED: AUGUST 29, 2026
These Terms of Use (“Terms”) govern your access to and use of cbxdirectlab.com (the “Site”), operated by Crisis Box LLC, doing business as Crisis Box Direct Lab (“Crisis Box,” “we,” “us,” or “our”).
By accessing the Site, requesting laboratory testing, or continuing to a third-party ordering platform, you acknowledge that you have read and agree to these Terms. If you do not agree, do not use the Site or request testing.
- ABOUT CRISIS BOX DIRECT LAB
Crisis Box Direct Lab provides educational information and administrative access to independent laboratory testing services.
Crisis Box Direct Lab is not a clinical laboratory, physician practice, medical provider, healthcare facility, or emergency service. Crisis Box Direct Lab does not perform laboratory testing and does not provide medical diagnosis, treatment, medical advice, prescriptions, result interpretation, or ongoing medical care.
Laboratory orders, when required, may be reviewed and authorized by an independent licensed healthcare provider through Evexia Diagnostics or another authorized provider network. That authorization is limited to facilitating the laboratory order and does not constitute a medical consultation with Crisis Box Direct Lab.
Testing is performed by the independent laboratory identified on the laboratory requisition and final report.
- ELIGIBILITY
Unless expressly approved through an appropriate minor-consent process, you must be at least 18 years old to request testing.
You may request testing only for yourself and must provide complete, current, and accurate information. You may not submit an order using another person’s identity or request testing for another person without appropriate legal authority and prior written approval.
Testing is available only in eligible United States jurisdictions and is subject to the requirements of Evexia Diagnostics, the independent ordering provider, the performing laboratory, and applicable law.
Access to testing is not guaranteed. A test request may be declined, canceled, or modified when required by law, provider review, laboratory availability, geographic restrictions, specimen requirements, or safety considerations.
- GOVERNMENT HEALTHCARE PROGRAMS AND INSURANCE
Testing offered through Evexia Diagnostics is self-pay and is not submitted to insurance.
Under Evexia’s current eligibility requirements, services may not be available to recipients or beneficiaries of Medicare, Medicaid, or another governmental healthcare program. You must not submit these charges to any governmental or commercial insurance plan for reimbursement.
You are responsible for confirming your eligibility before continuing with an order.
- NO MEDICAL ADVICE OR EMERGENCY SERVICES
Content on the Site is provided for general educational and informational purposes only. It is not a substitute for professional medical advice, diagnosis, treatment, or an examination by a qualified healthcare provider.
Do not use this Site or any laboratory testing service offered through it for an emergency or urgent medical concern. If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention.
Do not begin, discontinue, or change medication, supplements, diet, or treatment based solely on information from the Site or laboratory results.
- LABORATORY TESTING AND RESULTS
Laboratory results must be evaluated together with symptoms, medical history, medications, supplements, physical findings, and other relevant clinical information.
An abnormal result does not independently establish a diagnosis. A result within the laboratory reference range does not exclude a medical condition. False-positive, false-negative, inconclusive, delayed, or rejected results may occur.
Crisis Box Direct Lab does not guarantee that a test is appropriate for your circumstances or that testing will identify, predict, prevent, diagnose, or exclude any disease or condition.
You are responsible for following all preparation, collection, transportation, scheduling, and identification instructions. Failure to follow instructions may affect the accuracy of the result or cause the specimen to be rejected.
- THIRD-PARTY SERVICES
Ordering, provider authorization, payment, specimen collection, kit fulfillment, laboratory analysis, and delivery of results may be provided by independent third parties, including Evexia Diagnostics, an independent physician network, laboratory companies, collection centers, payment processors, and shipping carriers.
These third parties are separate organizations with their own terms, privacy practices, eligibility requirements, fees, and responsibilities. Crisis Box Direct Lab does not control their clinical decisions, laboratory procedures, processing times, platform availability, or shipping operations.
- PRICES, PAYMENT, AND ADDITIONAL FEES
Prices and test availability may change without notice before an order is completed.
Depending on the test, separate provider-authorization, collection, shipping, processing, reflex, cascading, or replacement fees may apply. Applicable charges are subject to the terms and authorizations presented through the ordering platform.
Payments completed through an Evexia Diagnostics Promo Page are processed according to Evexia’s payment terms. Crisis Box Direct Lab does not collect or store complete payment-card information from those transactions.
Testing is self-pay. No representation is made that any expense will qualify for insurance, HSA, FSA, tax, or reimbursement purposes.
- CANCELLATIONS, REFUNDS, AND REPLACEMENTS
All cancellations, refunds, specimen recollections, and kit replacements are governed by our Refund, Cancellation & Replacement Policy and any applicable Evexia or laboratory policy.
Submission of a cancellation request does not mean that an order has been canceled. A cancellation is effective only after written confirmation.
- ELECTRONIC COMMUNICATIONS
You consent to receive administrative communications relating to your request, including order status, eligibility, requisition availability, specimen instructions, and service notices.
Standard email is not intended for medical records, laboratory results, identification documents, or sensitive health information. Do not send sensitive health information to orders@cbxdirectlab.com.
Marketing communications require separate consent and may be withdrawn at any time.
- ACCEPTABLE USE
You may not misuse the Site, interfere with its operation, submit false information, impersonate another person, attempt unauthorized access, copy protected content, or use the Site for unlawful, fraudulent, or harmful purposes.
All Site content, branding, graphics, descriptions, and original materials are owned by or licensed to Crisis Box LLC unless otherwise stated.
- WEBSITE AND THIRD-PARTY DISCLAIMER
The Site and its content are provided on an “as available” basis. To the extent permitted by law, Crisis Box Direct Lab does not guarantee uninterrupted availability, error-free content, specific processing times, laboratory availability, or a particular testing outcome.
To the fullest extent permitted by applicable law, Crisis Box Direct Lab is not responsible for delays, acts, omissions, technical failures, specimen loss, laboratory errors, provider decisions, shipping events, or other actions controlled by independent third parties.
Nothing in these Terms limits any consumer right that cannot legally be waived.
- CHANGES TO THESE TERMS
We may update these Terms as our services, vendors, or legal obligations change. The updated version will be posted with a revised “Last Updated” date. Material changes will apply prospectively unless otherwise required by law.
- GOVERNING LAW
These Terms are governed by the laws of the State of Florida, without limiting any non-waivable consumer protection rights that apply where you reside.
